If a patient is under eighteen (18) years of age, the patient must designate a Caregiver. Patients must provide a valid copy of New Mexico State ID, such as a driver’s licence or New Mexico-issued I.D. Card. Patients must provide proof of residency, such as bank statements or correspondence with a state department within the last 3 months, or a rental/mortgage agreement. Patients must be evaluated, in person, by a certified physician. Physicians must establish a bona fide patient relationship. Patients must be diagnosed with a Qualifying Condition. Patients must submit an application to the New Mexico Department of Health Medical Cannabis Program.
