What documents do I need for a Utah Cannabis ID Card?

After being certified by a qualified physician and diagnosed with a qualifying condition, patients can apply for a Medical Marijuana Card.

Proposition 2 authorizes Utah’s Department of Health to begin issuing Medical Marijuana Cards no later than March 1, 2020.

Patients need to be a citizen and resident of Utah. Driver’s licence, photographic I.D. produced by the state of Utah, recent correspondence with a state department or bank statement, or a rental/mortgage agreement are acceptable.

Medical records. Progress notes, letters proving diagnosis, medical images, full medical records, medical images, list of medications. Out-of-state ID is not accepted in Utah.

If a patient is under eighteen (18) years of age, an approved and authorized Caregiver can accompany minors for Medical Marijuana Treatment.
